Message from the Chairperson’s

The topic ‘Stroke & AI 2025’ is very timely for discussion in current comprehensive stroke care, and I thank the organisers, particularly, Dr Biplab Das for selecting this topic. Like many other fields where AI (Artificial Intelligence) is provoking discussions, the filed of stroke is increasingly stirring participants of workshops and conferences to discuss how AI is going to affect the management of patients with stroke.

Questions are asked? Can it take up the role of radiologists in diagnosing disabling stroke requiring thrombolysis, or large artery occlusion (I don’t know why people use the word ‘large vessel occlusion’ when all the time they mean artery) requiring thrombectomy. Will it reduce ‘door to needle’ or ‘door-to-puncture’ time and so on? Will it improve outcomes? And so on.

Well, as Ajit Issa writes: “For more than two hundred years, each wave of new technology and innovation has carried with it a familiar fear that machines would take away people’s livelihoods. During the Industrial Revolution, the mechanization of textile and manufacturing triggered fears of ‘technological unemployment’. This panic even gave rise to the famous Luddite movement in England, where textile workers destroyed looms, they thought would make their jobs obsolete.” In fact, industrialization generated vast new opportunities. Similarly, I don’t think that AI would make the jobs of radiologists obsolete. Rather it will open new opportunities for them as well as neurologists in way that we cannot imagine now.

Will AI improve outcomes? It may, but to examine this properly, we would need randomized controlled trials comparing stroke outcomes in AI-enabled stroke care facilities to the conventional ones. Obviously, cluster randomized trial with facility as a ‘cluster’ unit will be more practical than randomizing individual patients. AI-enabling will be done to those facilities randomized to intervention arm.

We need to prepare ourselves for such a trial. This conference is being organized by the “We Care Stroke” serves as a forum to discuss such a trial and discuss how well can we use the AI to improve the fate of patients with stroke. I hope the participants will have academically enriching and socially enjoyable experience. I wish this conference, “Stroke & AI2025 a resounding success.
